Chandler Hall, a non-profit, Quaker facility, provides high-quality care for older adults in Newtown, Bucks County, and offers a wide spectrum of services for individuals at all stages of the aging process including Community Living, Personal Care, Home Hospice, Independent Living, Respite Care, and Skilled Nursing Care. We are currently seeking a caring and compassionate LPN.

We are currently seeking experienced LPNs to provide direct care to meet the social, physical, recreational, emotional, and spiritual needs of our Home Hospice patients.

We are hiring for full-time, part-time, and per diem LPNs for our Hospice at Home patients.

The primary purpose of the licensed practical nurse position is to function within your scope of practice and as part of an interdisciplinary team to ensure the provision of care at the end of life. Position includes administration of medications, treatments and patient comfort measures according to care plan. In conjunction with the RN case manager, the LPN will reassess outcomes related to care plan through provision of visits in patient homes, provision of continuous care in the patient's home setting, or care in facilities. Patient care and outcomes will be clearly documented. You are responsible for working as an interdisciplinary team member in all hospice settings to include home, nursing home, and other facilities to include following both organizational and regulatory policies and procedures. Essential Functions

Provide direct care to patients as prescribed in the Interdisciplinary Plan of Care in order to maintain the highest level of comfort and quality of life.
Under the supervision of the RN, perform ongoing reassessment and revise initial written plan of care with Interdisciplinary collaboration as the needs and conditions of the patient's family change. Promotes the provision of comfort care to the dying as an integral component of nursing care.
Executes physician orders: accepts, accurately transcribes and carries out physician orders, with demonstrable understanding of the reasons, desired and adverse outcomes. Demonstrates knowledge of patient monitoring related to medications and treatments, addresses concerns to RN for resolution.
Administers medications and performs treatments per physician order and within the scope of practice for PA. state licensed practical nurse; follows accepted standards of nursing practice with attention to rationale, desired outcomes, adverse effects; provides timely and accurate documentation of med pass, documents reactions and changes, addresses issues to RN for resolution. Seeks assistance from peers for challenges with medication acceptance, seeks information from known reliable sources for knowledge deficits regarding new medications/treatments.
Maintains clinical competence with: demonstrates ability to use diagnostic and monitoring equipment [glucometer, bladder scanner, oximeter, sphygmomanometer, nebulizer, and concentrator] to accurately assess and treat patient medical conditions. Maintain clinical competence with pain and symptom management. Knowledgeable regarding storage and securing additional supplies and equipment.
Reassess patient and report to the RN, attending physician and appropriate interdisciplinary team members, response to treatments and medications as well as changes in the patient's physical or emotional condition
Document patient care reflecting nursing interventions, patient response to care, patient needs, problems, capabilities, limitations, and progress toward goals

Conforms with the requirements of the compliance program by understanding and adhering to Chandler Hall’s Compliance Program and Policies as they appear in the Code of Conduct and other compliance policies. Annually acknowledges review of the Code of Conduct by signature, and annually participates in corporate compliance education. Adheres to the policies and procedures covering Privacy and Security in compliance with HIPAA regulations.

Demonstrates initiative to continually elevate skills and competency; provides leadership to support skill development and effective service delivery for the team and agency.
