Registered Nurse – Case Manager

Position Duties and Responsibilities:

Proactively identifies and evaluates patients and families for case management from a variety of sources such as discharge/disposition planning, patient MTF in/out- processing, Nurse Advice Line usage, Pulmonary Nodule Registry, referrals, the Medical Evaluation Board (MEB) process, the healthcare system, employers and facility staff. Conducts systematic, on-going, thorough collection of patient’s physical, emotional, psychological, social and medical status and information via direct patient contact and other relevant sources such as professional and non-professional caregivers, medical records, family/caregiver interviews.
Develops an appropriate patient-specific plan of care to include short- and long-term goals, objectives, and actions. Coordinates, collaborates, and obtains approval of the plan among the patient, family/caregiver, primary provider, and other members of the healthcare team.
Guides the patient and family/care giver through the healthcare system, maximizing use of resources. Coordinates and executes the plan of care, optimizing access to appropriate services. Ensures necessary referrals are ordered by the appropriate discipline and coordinated. Serves as an advocate for, and ensures education is provided to, the patient and family/caregiver as required. Promotes adherence to treatment plans for improved healthcare outcomes.
Ensures coordination of care delivery processes, to include alternate healthcare settings and the home environment, for the purposes of enhancing the patient's health and wellness, safety, productivity, and quality of life, and for providing the most beneficial, cost-effective health care. Develops, utilizes and maintains a variety of military and community resources to optimize access to services and medical care. Ensures timely and appropriate provision of services.
Documents and updates the treatment plan as needed in accordance with existing or developing DHA, DoD, AF, local facility and other agency guidelines. Maintains documentation and data collection in accordance with DHA, DoD, AF, local facility and other specified agency guidelines. Conducts and/or participates in program evaluation as directed.
Monitoring and evaluation may include but is not limited to: patient’s adherence and response to the treatment plan; timeliness of patient and family/caregiver contact and follow-up; identification of variances, patterns or trends from established practice guidelines and/or standards; established outcome measurements; results of interventions, treatment delivery and timeliness of care; and utilization of resources. Monitors and evaluates the facility’s case management program per DHA, DoD, AF and local policies and guidelines.
Coordinates and participates in interdisciplinary team meetings, designated facility meetings, and Care Coordination meetings. Shares knowledge and experiences gained from own clinical nursing practice and education relevant to nursing and case management.
Participates in the orientation, education, and training of other staff in a non-supervisory role. May serve on committees, work groups, and task forces at the facility.
Must maintain a level of productivity and quality consistent with: complexity of the assignment; facility policies and guidelines; established principles, ethics and standards of practice of professional nursing; the Case Management Society of America (CMSA); American Accreditation Healthcare Commission/Utilization Review Accreditation Commission (URAC); CAMH; The Joint Commission (TJC); Unit Effectiveness Inspection (UEI); and other applicable DHA, DoD and Service specific guidance and policies. Must also comply with the Equal Employment Opportunity (EEO) Program, infection control and safety policies and procedures.
Follows applicable local MTF/AF/DoD/DHA instructions, policies and guidelines.
Completes medical record documentation and coding, and designated tracking logs and data reporting as required by local MTF/AF/DoD/DHA instructions, policies and guidance.
Completes all required electronic medical record training, MTF-specific orientation and training programs, and AF/DoD/DHA mandated Case Management training.
Shall maintain proper medical records on beneficiaries to whom treatment is provided in accordance with Air Force Instruction 41-210, Medical Record Administration (or subsequent DHA policy/guidance). Medical records completed by the contract service provider shall be subject to review by the MTF Commander or his/her designated representative, and a medical records review committee established by the MTF. The contract provider shall be notified of deficiencies in his/her entries into the medical records and shall be instructed to correct the deficiencies. All records generated in the performance of duties under this delivery order will remain the property of, and subject to exclusive control of, the Government.
Shall complete each outpatient encounter in the current electronic medical record (EMR) customarily by the end of the shift and under extraordinary circumstances within 72 hours. The HCW shall be responsible for completing patient records in accordance with DHA, AF regulations and MTF policies.
Shall use current EMR for keeping records, ordering of ancillary procedures, ordering of medications, writing doctor's order, and performing other required patient record functions at the discretion of the MTF. Contract provider shall receive EMR training prior to commencing services.
Shall not introduce new medical procedures or services without prior recommendation to, and written approval of, the MTF Commander and the contracting officer.
Shall use only those abbreviations as listed in the MTF's official abbreviation book.
Shall attend and participate in MTF orientation, refresher training, team conferences, professional staff conferences, multidisciplinary trauma committee meetings, and other appropriate professional activities as determined by the MTF Commander, or his/her designated representative, as workload permits during scheduled duty hours. Such participation may include active and full participation in all areas of the Quality Improvement, Risk Management and Utilization Management Programs.
Shall complete newcomer’s orientation and other command directed training within 30 days of initial assignment and maintain such training in a current status throughout the life of this contract.
Shall assist service members and/or veterans, family members and caregivers with receiving the most appropriate options and services to meet their complex health care needs. This includes, but is not limited to, acute, chronic, multiple, complex, catastrophic, or life-threatening illnesses; combat stress, residuals of traumatic brain injury; community adjustment; addictions and other health problems.
Shall coordinate care with multiple providers across all levels and sites of care.
Shall address psychosocial, as well as nursing and medical needs of patients and their families/caregivers, through participation in multidisciplinary patient care management practice.
Shall evaluate care and outcomes to ensure timely and appropriate provision of services.

Minimum Qualifications:

A graduate or higher-level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools.
Basic provider cardiopulmonary resuscitation (CPR) face-to-face course such as American Red Cross Basic Life Support health care provider course or equivalent.
Case Management experience is not required but is highly desirable. Must have at least two (2) years’ experience working as a Registered Nurse in clinical settings and be able to meet practice expectations as required by the task order. Must possess a mature command of speaking, writing, reading and understand the English language, and a strong working knowledge and skills to use a variety of computer systems. A strong working knowledge of the military health care system and computer documentation is highly desirable.
Board Certification is not required; however, possession of Commission on Case Management or American Nurse Credentialling Center Case Management Certification is desirable.
A current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States; and you must have passed the National Council Licensure Examination.
Must be a US Citizen.
