Practice facilitation, also referred to as quality improvement peer coach, is an approach to supporting improvement in primary care practices that focuses on building organizational capacity for continuous improvement. As a practice facilitator, you will establish a long-term relationship with your practices, becoming a resource for ongoing quality improvement (QI) and evidence translation. PCPF's are specially trained individuals who work with primary care practices to make meaningful changes designed to improve patients outcomes. Is accountable for facilitating the implementation of practice transformation including baseline assessment of current environment, and ongoing facilitation of practice improvement methodologies to ensure compliance with regulatory and NCQA standards. Dispute Resolution For day-to-day operations, the Patient Centered Transformation Facilitator (PCPF) will report to the Patient Centered Transformation Practice Lead who reports to the Manager of Outcomes. The PF will support the development and maintenance of the Patient Centered Medical Home accreditation, and when applicable the Patient Centered Specialty Practices. The PF will support all members of the care team and provide educational support for all quality initiatives tied to the practices they support. Practice locations will vary based on an ever-changing mix of practice locations. Completes other special assignments and projects as assigned, including but not limited to work on Commercial and Medicare quality contracts, Roster Management, Health and wellness campaigns.

Responsibilities

A day in the life of a Practice Facilitor at Hackensack Meridian Health includes:

  • Current quality initiatives include but not limited to; Quality Measure Management (Hedis and MIPS), Reporting and Dashboarding for quality measures, action planning with the physician practices, HMHMG Panel Management activities and Annual Wellness activities, Patient Centered Medical Home accreditation, Social Determinants of Health. These quality transformation projects are subject to change. Must display core traits and behaviors consistent with HMHN 5 Culture during the 90-day orientation period: Creative, Compassionate, Collaborative, Courageous, and Connected. Practice Facilitator supports the work of the Ambulatory Quality and Patient Safety (AQPS) team in creating and sustaining an environment that fosters team member engagement. Coaches and supports assigned practice members as defined by the manager. For assigned practices, serves as the primary liaison, teacher, coach, and facilitator for practices participating in practice transformation activities and facilitates improvement toward measurable outcomes with increasing practice independence over time. Works with practices to understand quality results, and develop plans to improve Assures auditing of practices to maintain compliance with quality initiatives CMS and NCQA standards. Maintain databases directly associated with operational quality role. Acts as a liaison between the Quality leadership and the Ambulatory practices. Facilities Quality Improvement tactics, mentoring physician practice team members and implementing action planning based on outcome measures. Participate in the planning, development, presentation, and delivery of educational programs in support of CMS, NCQA PCMH and value based commercial payer standards. Review patient satisfaction results with practice and develop action plans as needed.
  • The Practice Facilitator will:
  • Act as mentor and provide training to assigned practices on the contents of specific transformational and quality improvements projects.
  • Will meet role competency requirements within 90 days of hire (Practice Facilitator Training Competency Checklist).
  • As a peer mentor the Facilitator is required to display evidence of core traits and behaviors consistent with the 5 HMH Cultural Beliefs during the 90-day orientation period: Creative, Compassionate, Collaborative, Courageous, and Connected (Practice Facilitator Training Competency Checklist)
  • Speak with confidence and exhibit advanced communication skills while delivering educational content to providers, managers and team members.
  • High degree of emotional intelligence is needed for this role. self-awareness self-management social awareness relationship management.
  • Assist practices in redesigning clinical documentation and or workflow to support desired changes.
  • Mentor staff modify clinical documentation and or process to ensure sustainability of changes.
  • Clinical documentation expertise - support process improvement tactics to optimize EMR clinical data capture to support of regulatory quality measures
  • Identify exemplary clinical documentation and processes in your practices and spread them to others.
  • Identify resources practices need to implement improvements
  • Mentor practice teams to integrate process improvement workflows occurring within them into a cohesive whole.
  • PCMH Practice audit quarterly: assure practice is maintaining NCQA standards.
  • Work with team to acquire recognition of excellence in NCQA Diabetes and Heart Stroke
  • Submit quarterly and annual workbooks to NCQA to re-credential and maintain recognition
  • Review monthly patient satisfaction results and work with the practice to improve through the development of action plans.
  • Document competency of team members in Regional Action Plans
  • Assist team in gaining CMS EDIM access as needed.
  • Report eCQMs (outcomes measures).
  • Monthly reporting of outcomes measures to assigned practices. Action planning based on measure findings.
  • Report operational quality quarterly.
  • Answer questions about practice's quality improvement efforts.
  • Each quarter, the PF will report information (action planning) on what the practice is doing to improve those eCQMs (outcomes measures).
  • Display traits and behaviors necessary to form and maintain a long-term relationship with your practices.
  • Display traits and behaviors consistent with HMHN 5 Culture: Creative, Compassionate, Collaborative, Courageous, and Connected.

Qualifications

Education, Knowledge, Skills and Abilities Required:

  • 5 years of healthcare experience - LPN or CMA (Certified Medical Assistant) in physician practices or practice management in a Physician Practice or related clinical role Health Care facility is required.
  • Need to drive and have daily access to a car. Travel required between assigned practices is required. May need to cover any practice within the Medical Group depending on organizational need.

Education, Knowledge, Skills and Abilities Preferred:

  • Bachelor's degree in any Health Care related field of study preferred
  • Master's degree in any Health Care related field of study preferred
  • NCQA PCMH Content Expert Certification preferred
  • Proficient use of google sheets, forms, and slides.
  • Ability to present quality outcomes using effective communication skills. Skilled communication is essential for the PF in order to maintain effective and sensitive relationships with the members of the care team.
  • Requires a comfort level using EMR and clinical data, clinical reporting and analytics tools such as clinical databases.
  • Excellent clinical skills, teaching skills, interpersonal skills, analytically minded, able to learn systems quickly.

Licenses and Certifications Preferred:

  • Licensed Practical Nurse or Certified Medical Assistant or Certified Clinical Medical Assistant or Certified Registered Medical Assistant or National Certified Medical Assistant or Clinical Medical Assistant Certificate or Nationally Registered Certified Medical Assistant or Registered Clinical Medical Assistant Specialist or Certified Registered Medical Assistant.
