General Purpose of Job:

Position responsible for impact on the lives of member's with complex and/or chronic conditions. The case manager role includes identification, assessment and interventions designed to optimize health outcomes, prevent acute exacerbations, manage the progression of chronic illness, and improve the quality of life. Activities include connecting with member on phone, in person, at the member's home, in the hospital and in the physician's office-essentially however and wherever the member needs assistance to improve the member's health and coordinate care.

Essential Duties:

This job description is intended to cover the minimum essential duties assigned on a regular basis. Associates may be asked to perform additional duties as assigned by their leader. Leadership has the right to alter or modify the duties of the position.

  • Works with high risk members telephonically, as well as provides face-to face interactions with members and their physicians, based on the members' needs.
  • Conducts comprehensive assessments that include the medical, behavioral, pharmaceutical and social needs of the patient, identifies gaps in care and barriers to attaining improved health.
  • With the member, physician and care team, creates and implements a personalized care plan that will address the identified needs, remove barriers and improve the health of the member.
  • Educates and builds member confidence and motivation by enabling the member to more confidently and effectively self-manage their complex or chronic conditions.
  • Coordinates care by acting as the contact point, advocate and resource for the member, and their family and their physician, building effective relationships through trust, respect and communication.
  • In collaboration with the member, primary care provider and care team, continually assesses the patient knowledge of their clinical condition(s) and provides education based on the member's unique learning style.

Ability to measure outcomes and evaluate the effectiveness of interventions for the member.

Job Requirements General Requirements Current unrestricted license to practice as a Registered Nurse in the State of Michigan Valid unrestricted driver's license Certification in Case Management preferred Work Experience Minimum of 3 years of nursing experience, preferably in home health, ambulatory care, community public health or case management, coordinating care across multiple settings and with multiple providers Knowledge and experience teaching self-management Experience with the case and disease management processes of assessment, planning, and intervention Motivational interviewing experience Experience in a managed health care setting preferred Education Graduate from an accredited nursing program Bachelor of Science Degree in Nursing, or Bachelor Degree in a health related field preferred Specialized Knowledge and Skills Team members possess enthusiasm, a strong work ethic, integrity, who are kind and empathetic, have a sense of humor, and who enjoy working in a collaborative, team-oriented system Excellent time management, project management, and prioritizing skills Demonstrate ability to work in stressful situations and manage conflict Excellent telephonic communication skills Strong written, verbal communication and decision making skills Creative problem solving with the ability to use community and network resources to meet members needs. Proficient with electronic project/calendar/email programs and database/spread sheet/word processing programs. Fluency in a foreign language preferred
