This is a per diem/on call position supporting the PACE Irvington site.

The Primary Care Manager RN provides care management, care coordination, skilled care and services for at-risk frail elder patients with complex medical and psychosocial issues. Population-based disease management approaches and application of clinical protocols are integral to the program. This position is a core member of the patient centered PACE interdisciplinary team (IDT).

Required qualifications:

• Upon hire: Oregon Registered Nurse License
• Within 30 days of hire: National Provider BLS - American Heart Association
• 2 years RN experience in PACE, home health, skilled nursing facility, community health, hospice nursing or related experience

Preferred qualifications:

• Bachelor's Degree in Nursing
• Upon hire: Current recognition by a national professional organization in care management, geriatrics or hospice

About the Team

Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.

HCC provides a full range of services and support for people of all ages, especially those facing chronic or life-limiting illnesses. Our core competencies include restorative care, longitudinal care and care through the end of life.
