40 hours per week; Monday - Friday 8AM - 4:30; with rotating holidays and emergencies on call.

Location

This position will travel to see patients in the following counties - Cherokee, Macon, Graham, Jackson, Clay and Swain . Mileage Reimbursement Included.

Pay Range $28.64 - $40.32 per hour

Position Summary:

Provides nursing care and instruct/supervise the LPN and Hospice Aide staff in the delivery of nursing care to clients.

Performs patient visits, 4 to 6 per day depending on location and acuity of patients served. May perform 2-4 admissions depending on length of shift and complexity of patient. At times they may receive assignments that are a combination of admissions & patient visits – adjusted accordingly depending on the mix of each.

Essential Duties & Responsibilities:

Practice within the scope of license, certification or registration, where applicable, and as required by law and regulation (See Components of Nursing Practice for the Registered Nurse)
Assess the patient's physical and mental health including the patient's reaction to illnesses and treatment regimens
Record and report the results of the nursing assessment
Plan, initiate, deliver, and evaluate appropriate nursing acts
Teach, assign, delegate to or supervise other personnel in implementing the treatment regimen
Collaborate with other healthcare providers to develop the appropriate patient and family-centered nursing plan of care for the patient and the family
Ensure coordinated implementation of the nursing plan of care
Implement the treatment and pharmaceutical regimen prescribed by any person authorized by State law to prescribe the regimen
Provide teaching and counseling about the patient's health, disease progression, and symptom management
Report and record the plan for care, nursing care given, and the patient's response to that care
Supervise, teach, and evaluate those who perform or are preparing to perform nursing functions
Provide for the maintenance of safe and effective nursing care, whether rendered directly or indirectly (by unlicensed staff, patients and families)

Knowledge, Skills & Abilities:

Must be willing and able to care for patients with infectious disease processes unless approved accommodations
Knowledge of Federal, State and Local regulations and Medicare Conditions of Participation
Knowledge of HIPAA privacy rules and regulations
Knowledge of electronic medical records systems
Basic data entry skills

Excellent written and oral communication skills
Ability to work under pressure to meet deadlines
Ability to take initiative and utilize innovative techniques and ingenuity

Minimum Qualifications:

Experience:

Two years of clinical experience preferred

Licensures & Certifications: (if applicable)

Valid NC RN license
Valid Driver’s License and proof of automobile insurance

Summary of Physical Requirements:

Walking, standing, sitting, bending, reaching, twisting, crouching/squatting, kneeling
Flexion/Extension of arms/wrist
Hand dexterity to include gripping, pushing/pulling and writing
Lifting/carrying of 50 lbs. often
