Job Description:
- Conduct initial patient assessments to develop individualized care plans tailored to the patient's needs and preferences.
- Provide skilled nursing care, including wound care, medication management, and symptom management, in the comfort of patients' homes.
- Educate patients and their families on disease management, medication administration, and other aspects of care to promote independence and improve outcomes.
- Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to coordinate comprehensive care for patients in the home setting.
- Monitor patients' health status and response to treatment, adjusting care plans as needed to optimize outcomes and ensure patient safety.
- Document all care provided accurately and timely, maintaining thorough and up-to-date patient records in compliance with regulatory standards and agency policies.

RN Home care experience - 2+ years
