Professional Supervisory Team Lead - Licensed Counselor, RN, OT

M-F, 8-4:30

The Licensed Mental Health Professional (LMHP) Supervisory Team Lead provides care to clients utilizing the Robert Young Center (RYC) principles of Trauma Informed Care. The LMHP Supervisory Team Lead provides direction for a case management team, delivers comprehensive community based mental health services, and maintains accurate clinical documentation. The LMHP Supervisory Team Lead may be recognized as a Licensed Practitioner of the Healing Arts under Medicaid Community Mental Health Services Rule 132. The LMHP may train students assigned to the department. The LMHP Supervisory Team Lead demonstrates Trinity's principles and values when interacting with clients, families, visitors, physicians and coworkers.

Responsibilities

Clinical Leadership

  • Coordinates the multi-disciplinary process on behalf of the individual consumer for an assigned group of Mental Health Professionals (MHP) and other community support staff.
  • Assigns referrals, reviews clinical activities, and may certify the diagnosis and recommended treatment plan.
  • Assures the timely and accurate completion of treatment plans, annual re-assessments, and daily progress notes.
  • Ensures that all team members meet / exceed established productivity standards.
  • Provides clinical supervision
  • Provides quantitative information about each MHP's performance to the CSP Clinical Manager for annual performance reviews.
  • Completes mental health intakes for CSP referrals.
  • May serve as an LPHA with proper licensure

Care Coordination

  • Assures that all clients receive clinically sound triage/referral and ongoing services for behavioral health needs.
  • Performs diagnostic assessments and collects diagnostic information to determine an individualized treatment plan for the client.
  • Plans treatment modalities, coordinates treatment plans and follows clients through the treatment process
  • Requests consultation for clients that includes providers, social workers, and nursing staff when necessary.
  • Evaluate the client's treatment plan through follow-up assessment and makes adjustments based on clinical findings.
  • Identifies available support systems to meet the needs of each individual client.
  • Delivers high quality care appropriate to the age and educational level of the client.
  • Responds to crises situations for chronically mentally ill clients in community locations and the emergency department and provides emergency assessment, recommendations, and crises intervention.
  • Provides individual, family and group therapy
  • Participates in the RYC on-call schedule for emergency response after hours, weekends, and holidays.

Clinical Excellence

  • Maintains accurate, concise, current clinical documentation. This includes but is not limited to all phone conversations, requests from the consumer, changes in consumer condition, and all consultations with members of the health care team.
  • Integrates ethical principles, evidence based practice, and research findings into all areas of practice.
  • Establishes and maintains good working relationships with the medical, educational, and the social services community.

Client Education

  • Utilizes a holistic approach to determine teaching methods appropriate to the individual client.
  • Provides education to consumers regarding their illness and how it relates to employment, and supplements information provided by health care professionals.
  • Evaluate the client's understanding of information provided and ability to follow through.

Qualifications

Education:

  • Master's degree in Social Work, Counseling, Marriage and Family Therapy or a related field from an accredited program; or
  • An individual possessing at least a master's degree in counseling and guidance, rehabilitation counseling, social work, vocational counseling, psychology, pastoral counseling, or family therapy or related field, who has successfully completed a practicum or internship that included a minimum of 1,000 hours of supervised direct service, or who has one year of clinical experience under the supervision of a QMHP.

License(s)/Certification(s):

  • Licensed in Illinois with - Licensed Clinical Social Worker, Licensed Clinical Professional Counselor, Licensed Marriage and Family Therapist with two years' experience in a community mental health setting

OR

  • A registered nurse (RN) with at least one year of clinical experience in a mental health setting or who possesses a master's degree in psychiatric nursing

OR

  • An occupational therapist (OT)
  • Valid Illinois driver's license.
  • Basic Life Support (BLS) certification.
  • Crisis Prevention Institute (CPI) training within 6 months of hire.
  • Obtain certification within 2 months of hire and re-certify annually

Knowledge/Skills/Abilities:

  • Demonstrated ability to apply critical thinking and problem-solving abilities to a wide range of client situations.
  • Able to accurately collect and assess information, understand ethical standards of behavioral healthcare, and utilize teaching/learning theories.
  • Demonstrated excellent communication skills (orally and in writing) and presents self in a professional manner.
  • Able to establish effective consultative relationship with health care providers, outside agencies, co-workers, and the community.
  • Able to work sensitively with clients.
  • Able to handle multiple priorities, maintain composure under stress, and adapt to a fast paced changing environment.
  • Proficiency in the use of computer programs including Microsoft Office and Electronic Medical Record (EMR) applications
