RESPONSIBILITIES


Position Summary: To demonstrate support of the Company's values by providing Navigation services to clients and families to help in the coordination of care and the management of a patient's individual health needs through and beyond the course of treatment. The Lay Navigator will be assigned patients to perform initial data collection which will be documented in Onco EMR. The Lay Navigator will assign the patient to the RN Patient Care Manager. They will educate the patient and family about how to contact Care Management and the services that will be provided by their RN Care Manager. If at the time of the assessment the patient needs community resources the Lay Navigator will assist the patient to obtain the needed resources, all of which will be documented in the EMR. If the patient has any clinical issues while on the call the navigator, will discuss with the navigation nurse supervisor and direct the issues with the care management triage RN. The Lay Navigator will support the mission of the Company and the Care Management department.


QUALIFICATIONS


1. Education: High school diploma required
2. Previous Experience:
* One (1) year of experience in the healthcare field or clinic setting with knowledge of oncology preferred.
* Procedure entry with experience in insurance and medical terminology preferred.

* Self-Management: Effectively manages own time, conflicting priorities, self, stress, and professional development. Self-motivated and self-starter with ability work independently with limited supervision. Ability to work remotely effectively as required.
* Must be able to work effectively in a fast-paced, multi-site environment with demonstrated ability to juggle competing priorities and demands from a variety of stakeholders and sites.
* Computer Skills:
* Proficiency in MS Office Word, Excel, Power Point, and Outlook required.
