Sentara Health is seeking to hire a qualified individual to join our team as a Team Coordinator, Care Coordination, Population Health.

Position Status: Full-time, Day Shift

Position Location: This is a hybrid position with 3 days on site required at our new corporate office in Virginia Beach.

Standard Working Hours: 8:00AM to 5:00PM

Minimum Requirements:

  • Required: Bachelor's degree

  • Required: Patient care - 3 years

  • Preferred: Population Health

  • Preferred: Experience in an ambulatory primary care setting

  • Preferred: Registered Nurse

  • In lieu of bachelor's degree, will consider seven years of experience. Certified Health Coach required within two years of hire. Experience with Microsoft Office suite, Visio and Project preferred.

Position Overview:

The Team Coordinator, Care Coordination, Population Health will be responsible for facilitating implementation, organization and providing ongoing oversight for the Emergency Room Discharge program for the SQCN and SACO population. This initiative is designed to improve the care for the populations we are accountable for through value-based contracts and reduce unnecessary costs of care such as low acuity emergency room visits and frequent emergency room visits. The Team Coordinator will work with the population health team to improve transitions of care between emergency room and primary care, provide targeted patient centered education, and increase care-coordination capabilities. Responsibilities include communicating with external and internal partners, tracking tasks and monitoring outcomes and progress; assisting with developing and distributing communication to participating providers and practices; assisting with documentation of workflows and processes. The Team Coordinator will oversee staff assigned to care for patients discharged from the emergency room. The Population Health Team Coordinator must be able to work within a matrix structure having a direct reporting relationship to the SQCN Manager, Clinical Performance, but also accept and respond to assignments that are initiatives led by other leaders under the population health umbrella.

The Population Health Team Coordinator will be accountable for monitoring and engaging Population Health team members to improve financial and quality performance, patient experience, safety, and access and provides educational services to Population Health staff and participates in the work activities of the assigned team and provides care coordination services as needed.

Bachelor's degree in health education or health related field and three years patient care.





Population Health experience preferred.



Work in an ambulatory primary care setting preferred.
