SUMMARY

Family Service League is seeking a full-time Social Worker for the DASH (Diagnostic, Assessment, and Stabilization Hub) Crisis Center. The DASH Crisis Center is a 24 hour per day, 7 day per week outpatient crisis stabilization program that includes a 24-hour crisis hotline and mobile response team. The Social Worker will be responsible for conducting assessments, providing crisis intervention, and following up with clients during their stabilization period.

RESPONSIBILITIES

  • Conduct intake assessments on adults, adolescents, and children with the full range of DSM diagnoses (including both mental health and substance use disorders) in an acute care, outpatient setting or in the field as part of the Mobile Crisis Team.
  • Provide telephonic and in person crisis intervention.
  • Completion of all clinical documentation in accordance with NYS Office of Mental Health in an electronic medical record.
  • Provide linkage and follow-up to clients during their stabilization period and immediately following.
  • Coordination with outside providers and collateral contacts.
  • Attendance at routine supervision/team meeting.
  • Coordinate treatment with multidisciplinary team.
  • Evening and weekend hours required.
  • Complete training and maintain proficiency with de-escalation and physical restraint skills.
  • Establish and maintain positive working relationships with others both internally and externally to achieve the goals of the organization.
  • Communicate effectively, both verbally and in writing.
  • Foster teamwork, work cooperatively and effectively with others to set goals, resolve problems, and make decisions.
  • Assess situations to determine the importance, urgency, and risks, and make clear decisions which are timely and in the best interests of the organization.
  • Set priorities, develop a work schedule, monitor progress towards goals, and track details, data, information, and activities.
  • Assess problem situations to identify causes, gather and process relevant information, generate possible solutions, and make recommendations and/or resolve the problem.


QUALIFICATIONS

Master’s Degree required.

LMSW, LMHC, LMFT or related licensed clinical degree required.

Minimum of 6 months of experience in a behavioral health setting required. Clinical experience preferred.

Proficient computer skills, including Microsoft Office required. Experience with an Electronic Health Record preferred.

Excellent interpersonal and verbal and written communication skills required.

Ability to problem solve, multitask, make clear decisions and work as part of a team required.

Valid and clean NYS Driver’s License and ability to safely operate a motor vehicle is required for the Mobile Response Team.

Bilingual in Spanish preferred.

Evening and weekend hours may be required.

$50000 - $56000 / year
