Assist patients who have complex psychosocial needs. Serves as a resource to staff and community regarding psychosocial issues and community resources. Participates in Hospital and departmental planning committees and teams.




valuates and assesses the social, cognitive, emotional, behavioral, and environmental status of patients/residents upon referral from patients/families, physicians or patient care providers.


Conduct complex discharge planning for patients/residents, including assessing and responding to needs for resource or extended care placement. Directs patients in obtaining medication assistance resources, transportation, medical supplies and adaptive devices.


Provides ongoing counseling and emotional/social support in relation to health/social needs, complex family dynamics, and plan of care to enable them to cope with difficulties and properly utilize services.


Documents interventions and updates care plans as prescribed by regulatory agencies, and as problem situations occur.


Participates in ongoing evaluation of patients’/ residents’ needs and goals through multi-disciplinary care planning to ensure a timely discharge and to provide appropriate linkage with post-discharge care providers.


Provides assistance with and acts as a resource to patients/ residents and families concerning public aid and legal documents, including but not limited to living wills, Do Not Resuscitate (DNRs), and power of attorney for health care.


Provides intervention in cases involving child abuse/neglect, domestic violence, elder abuse/neglect, institutional abuse and sexual assault.


Serves as the lead in obtaining legal guardianship and competency determinations as well as adoption related situations. Completes and disseminates all necessary legal and clinical documentation required.


Consults with and keeps director informed of activities, needs, and problems.


Assists in care and maintenance of department equipment and supplies.


Serves as a resource to Medical Center staff and the community by providing in-service education programs and psychosocial information as requested.


Attends meetings and participates on committees and task forces as directed.


Orients and supervises field placement students in the completion of their practicum.


Assists in verifying insurance coverage for patients/residents and provides clinical updates to insurance companies.

Education and/or Experience

Master’s degree in Social Work, or related field, from a school approved by the Council on Social Work Education (CSWE) required


Minimum two years experience in hospital social work or relevant work in the healthcare field is preferred.


Computer Skills

Proficient in Microsoft Office software (Word and Excel).


Certificates, Licenses, Registrations

Current State of Illinois license as a Licensed Social Worker (LSW) or as a Licensed Clinical Social Worker (LCSW). LCSW is preferred.
