RN Nurse Evaluator- Traumatic Brain Injury Sunnyview Rehab Hospital-Full-Time
Sunnyview Rehabilitation Hospital, a state-of-the-art physical rehabilitation hospital located in Schenectady, New York, is in need of a Nurse Evaluator (NE) for the Traumatic Brain Injury (TBI) and Nursing Home Transition and Diversion (NHTD) Home and Community Based Medicaid Waivers. What you will do:
* This employee will support the mission and functions of the Capital Region Regional Resource Development Center for the NHTD/TBI Waiver Program.
Responsibilities:
* Assist the Lead Regional Resource Development Specialists & Assistant Regional Resource Development Specialists in assessing potential participants as to their medical/safety needs & appropriateness of referral and eligibility for Waiver services.
* Serve as back up during Lead RRDS' absence for service plan approval & Notice of Decision (NOD) authorization.
* Assist the RRDC staff & Provider Agencies by reviewing & administering eligibility assessments in timely manner & as appropriate including UAS-NY Assessments
* Review all medically based incident reports & sevice plans as needed and/or as requested by RRDC staff
* Provide trainings, technical assistance & education to waiver providers & RRDC staff on clinical aspects of the waiver program & share expertise with other NE's
* Act as a liaison between participants, families, service provider agencies, and all applicable community/government entities re: medical/safety needs & issues
What you will need:
* New York State RN License.
* Associates Degree in Nursing. BSN Preferred
* 3+ years RN experience in the delivery of home and community based services
* Strong interpersonal skills with ability to maintain the highest level of customer service while working in a fast paced environment
* Critical thinking and problem solving skills
* Passion to learn, grow and develop in the nursing field
