There are many outdoor activities to enjoy at our nationally recognized Pacesetter Park, along with music and entertainment found at Centennial Terrace and Downtown Sylvania, and the LPGA annual tournament.  The Flower Hospital campus is a 311-bed facility that houses the Hickman Cancer Center, ER Trauma Center, and is the regional leader for inpatient adult psychiatric services.  The campus includes, the Goerlich Center, which provides residence for patients with Alzheimer’s and Dementia, the Ebeid Center our hospice facility.


 


POSITION SUMMARY


The Transition Care Manager is responsible for managing all aspects of patient transitions to and from the Flower Rehab Center.  This role involves extensive medical record review and interaction with physicians, nursing, therapists, leadership, coding and clinical documentation specialists, patients, caregivers and the community.  Active participation in team meetings and education of all aforementioned roles regarding the Inpatient Rehabiltitation Facility admission requirements and processes as well as the expectations of the FRC unit.

REQUIRED QUALIFICATIONS


Education: Registered Nurse from an accredited school of nursing or a bacheolor’s prepared allied health professional.


Skills: Strong verbal and non-verbal communication and persuation skills both at patient and healthcare professional levels.  Ability to solve interdisciplinary problems in a team environment.  Abiity to input, retrieve and analyze relevant EMR data.  Mid-level Microsoft Word and Excel skills.


Years of Experience: Minimum of 2 years clinical practice experience in a patient care setting.


License: Registered Nursed Licensed in the state of Ohio or Licensed or Registered Allied Health Professional


Certification: CPR


 


PREFERRED QUALIFICATIONS


Education: Bachelors, Masters or Doctorate degree in a nursing or allied health profession.


Skills: Ability to work with a variety of internal/external customers and healthcare providers effectively and diplomatically.


Years of Experience: 3-5 years clinical practice experience in a patient care setting. 


License: N/A


Certification: Certified Rehabilitation Registered Nurse (CRRN), Allied Health Professional certification in IRF related diagnoses and/or patient management


 


ADDITIONAL EXPERIENCE


Previous experience with marketing, case management and/or inpatient rehab.
