We are looking for Primary Care Physicians that share our passion for improving the life of Seniors; we have practice opportunities Nationwide from Florida to Arizona and are experiencing rapid growth. Our physicians lead our care team and take a hands-on approach to population health. We address patient needs inside the exam room and beyond, to understand how social determinates of health impact outcomes. We provide a comprehensive Care Management program to not only help patients manage chronic disease, but to address their needs outside of the exam room, including food insecurity, social isolation and transportation issues. With a greater focus on social needs and using social determinants as a predictor for better health, our model brings with it an emphasis on mental health and social well-being.

Required Qualifications

* Current unrestricted medical license in the state of practice or willing to obtain a medical licenses in state prior to employment, and eligible and willing to obtain licenses in other states in the region of assignment, as required
* Graduate of accredited MD or DO degree; successful completion of an accredited ACGME/AOA family/internal/geriatric medicine residency program
* This role is considered patient facing and is a part of CenterWell's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

Preferred Qualifications

* Board Certification or True Eligibility in Family Medicine, Internal Medicine or Geriatric Medicine with continued certification throughout employment. We recognize ABOM, AOA and ABPS certifications.
* Experience managing Medicare Advantage patients with understanding of Best Practice in a coordinated care/value based relationship environment
* Knowledge of Medicare guidelines and coverage
* Knowledge of HEDIS quality indicators
* Familiar with the Value Based Care Model

Additional Information

For this job, associates are required to be fully COVID vaccinated or undergo weekly COVID testing and wear a face covering while at work. The weekly testing will need to be done through an approved Humana vendor, and unvaccinated associates should follow all social distancing and masking protocols if they are required to come into a Humana facility or work outside of their home.
