We are seeking to appoint an enthusiastic, motivated, reliable Community Health Care Support Worker who is keen to deliver a range of quality care services to patients who have palliative care needs in their own homes throughout the night as part of the Community Health Support Team based at Civic Way, Ellesmere Port. The team visits people residing across the West Cheshire CCG footprint.
The vacancy is for full time or part time, working . Shifts are 9 hours per night including Bank holidays and weekends between the hours of 22.00 - 07.00.
As part of a friendly team, you should have experience of working in patients' homes and the ability to carry out tasks such as simple dressings, catheter care, personal care, recording blood sugars and have experience of providing care for palliative patients. This post requires travel across the Western Cheshire footprint and lone working, access to transport for work purposes is essential.
An enthusiasm to work as part of a team which delivers a high quality nursing services is a must for this role and you will also need to demonstrate excellent communication and interpersonal skills with the ability to be flexible, reliable and approachable.
CWP provides health and care services for local people, including mental health, learning disability, community physical health and all-age disability care - including the provision of three GP surgeries.

All new clinical support staff will be required to have completed or working towards the Care Certificate (CWP In-house 2 day training to be completed within 6 months of start date).
Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. You are encouraged to enrol for the DBS Update Service. A small annual fee of £13 per year applies.
New entrants to the NHS will commence on the minimum of the scale stated above.
