Description

The Institute for Family Health, a not-for- profit health care organization, is seeking a full time case manager to work as part of our interdisciplinary, patient centered are for the Homeless team. The Care for the Homeless program provides health care and social services to homeless people at several locations in Manhattan - including churches, small shelters, and a center for LGBTQ youth.

Responsibilities

The ideal case manager candidate is familiar with all public benefits - especially Medicaid. Experience in health care a plus. The case manager will work at multiple homeless program sites and will be responsible for outreach, engagement, assessing patient needs, accessing resources, assist patients in applying for public benefits, arranging for specialty care medical and other appointments, and coordinate all aspects of care with the health care team. The case manager will also provide accompaniments (escorts) to medical and social service appointments when needed.

Strong knowledge of public benefits desired, knowledge of resources for homeless people and the health care system, a plus.

Bachelor's degree required. Spanish speaking preferred.
