Job Summary

The Home Infusion Service Nurse (Home Visit Nurse) is a licensed professional who plans for, coordinates and provides individualized nursing  care to patients and families in their homes.  The provision of home infusion services reflect the mission, philosophy and standards of home infusion therapy care of the agency.  

 

Essential Functions

  • Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.


  • Performs patient care in a timely and efficient manner.  Meets productivity expectations.  Maintains a positive impact on customers (internal and external)
  • Reviews and revises plans of treatment, performs patient assessments as required by patient condition and or by agency policy.  Coordinates the discontinuation of IV services with clients in a complete, thorough and timely manner.  Paperwork  is submitted for data entry within the 48 hour guideline
  • Communicates with other members of the health care team on issues pertaining to client care needs, status updates and coordination of care issues
  • Interacts with co-workers, visitors, and other staff consistent with the core values of the Hospital.

Qualifications

Minimum Education and Experience Requirements:

Education:

RN with diploma, Associates or Bachelor’s degree in nursing

AND

Experience:

At least 2 years in the acute care setting or 1 year in home care, hospice or home infusion setting

 

Minimum Certifications, Registration or License Requirements:

Licensure in state of nursing practice

BLS

Driver’s License

Chemotherapy certification recommended

 

Conditions of Employment

Covid Vaccination is a requirement for employment at Jefferson for employees working at Jefferson’s clinical entities or at the University.  If you are not currently vaccinated you will be required to receive the vaccination prior to hire date if you are offered employment, unless you request and receive an approved medical or religious exemption from Jefferson.

Since this position will require the selected candidate to engage in regular contact with children in the form of care, guidance, supervision or training, you will be required to meet the requirements of the Child Protective Services Act,  including the Pennsylvania State Police criminal background check, the Pennsylvania Department of Welfare child abuse criminal background clearance, and an FBI finger print based federal criminal records check.  The selected candidate will not be allowed to commence working until all three (3) are completed satisfactorily.
