**Required Qualifications:**

+ Doctor of Medicine (M.D.) or Doctor of Osteopathy (D.O.)

+ Board certification or eligibility in Family or Internal Medicine

+ Active, unrestricted medical license in Texas

+ Valid DEA license

+ Bilingual (English/Spanish fluency
