The mission of the Colorado Coalition for the Homeless is to work collaboratively toward the prevention of homelessness and the creation of lasting solutions for homeless and at-risk families, children, and individuals throughout Colorado. The Coalition advocates for and provides a continuum of housing and a variety of services to improve the health, well-being, and stability of those it serves.

The Colorado Coalition for the Homeless seeks aProgram Managerto oversee a team of Case Managers in the Family Support Services [FSS] program to ensure appropriate and high-quality case management services to families participating in housing programs in accordance with CCH and regulatory guidelines; provide direct case management services as needed; serve as the primary liaison and collaborative partner between FSS Program and Renaissance Property Management Corporation (RPMC) (the real estate arm of CCH); collaborate with community agencies and sub-grantees to establish and maintain effective working relationships; serve as a member of the CCH and FSS Management Teams; maintain a working knowledge of Continuum of Care [CoC] HUD regulations for the Rapid Re-Housing Program, and general knowledge of permanent housing program/voucher requirements including but not limited to Department of Housing (DOH) and Denver Housing Authority (DHA).

Overall, this position contributes to a cross-functional team that uses a collaborative approach, critical thinking, and a healthy sense of humor to problem-solve crises and identify and implement proactive solutions to help children and families thrive. This position is located at 2111 Champa Street., Denver, Colorado 80205 and reports to the Director.

Essential Job Functions

  1. Provides ongoing supervision to FSS case managers to ensure staff follows acceptable standards, completes work in a timely and accurate manner within the CCH regulatory guidelines. This may specifically involve a review of the electronic health record, treatment plans, case plans, Homeless Management Information System (HMIS) case notes, and other documentation within case files.
  2. Provides crisis response support to case managers and clinical teams as appropriate and serves a pivotal role in guiding case conference planning activities.
  3. Completes required paperwork and ensures accurate and timely reporting to HUD, CCH administration, and funders. Also ensures supervisees meet all reporting requirements.
  4. Works to establish community collaboration promoting effective coordination and continuity of services to families participating in the FSS program, particularly those participating in the Rapid Rehousing program. Close collaboration is particularly important with local schools, community agencies, vocational services, etc.
  5. Participates in coordinated meetings with RPMC teams at various property sites throughout the Denver metro area, and serves as a primary communication bridge between FSS program and property.
  6. Works collaboratively with RPMC regarding community-building relations, activities, and events at assigned properties.
  7. Recruits and hires case management staff in cooperation with the FSS Team and completes performance reviews for direct reports.
  8. Oversees interns and volunteers if/when applicable.
  9. Assists with grants, Annual Progress Reports (APR's) and ensures that program data is collected efficiently and effectively so that it can be used in APR's and other program reports.
  10. Monitors the team and subgrantee budget spending, reviews and approves expenses, and assists with variance reports and check requests.
  11. Coordinates services with Stout Street Health Center, especially psychiatry, pediatric, and mental health services, and assists other CCH programs regarding family-related issues.
  12. Demonstrates core competency in HMIS and EHR medical records system.
  13. Carries a cell phone to assure accessibility and responsiveness to both emergent and non-emergent issues.
  14. Maintains a work environment that is marked by respect for others; that values inclusiveness and builds workforce diversity, and that fosters cooperation and teamwork.
  15. Record all services in a timely and complete fashion in the EHR.
  16. Maintain up-to-date records in EHR.
  17. Record telephone and other communications as needed in the EHR.
  18. Manage tasks and scanning as needed to have a complete and current chart.
  19. Maintain accurate information regarding client's insurance status.
  20. Utilize the EHR and any future upgrades as directed.
  21. Maintains current and complete COVID-19 vaccination(s), as defined by the Centers for Disease Control and Prevention and local health authorities.
  22. Performs other duties as assigned by management.

Qualifications Summary

  1. Bachelor's Degree in Human Services related field. Master's level preferred.

  2. A minimum of three years experience providing direct services to underserved populations preferred. Preference given to candidates with specialized knowledge of issues pertaining to homelessness and housing, families, and children.

  3. Previous management experience, working with a multi-disciplinary team strongly preferred.

  4. Bilingual (English/Spanish) preferred.
