Educational Requirements

Currently registered/licensed in the state, and in good standing with the Board.

Position Requirements

This position requires 100% local travel on a daily basis. A minimum of six months nursing experience within the past three to five years.

Per Diem Hospice LPN Position



We are seeking a highly skilled and experienced Licensed Practical Nurse (LPN) to join our growing hospice team, working under the direction and supervison of the Registered Nurse (RN).  



Patient's visits occur in the community, at home, or care facilities as appropriate. 



Responsibilies include, but not limited to:



Provides routine care for patients.



Prepares clinical and progress notes documenting outcomes of interventions.



Assist RN and/or physician in performing specialized duties related to end of life.



Assist RN in carrying out the plan of care.



Participates in regularly scheduled interdisciplinary team meetings to coordinate patient care, exchange information, problem solve, and receive staff support and in-service education as appropriate. 



Assists with orientation of new employees, as requested.



Meets or exceeds established productivity standards.



Take part in on-call rotation.
