RN/BSN - Sr. Patient Care Coordinator - Oncology Navigator (Lung/Hematology)


Reporting to the Administrative Director of the Asplundh Cancer Pavilion (ACP), and under the general direction of the Medical Director, the Nurse Navigator - will serve as advocate, educator, facilitator and guide for patients who are moving through the diagnostic imaging, biopsy and eventual treatment process at Jefferson Health.

  • Coordinates and facilitates the multi-disciplinary Services through consultation with patients in the diagnostic process, coordination of imaging and biopsy services, surgical Intervention and communication with physicians to coordinate surgical intervention and facilitate the transition of patients from the diagnostic to therapeutic phase of AH services to offer comprehensive, patient centered care.
  • Assist with the conduct of clinical trials via new patient education and ongoing symptom support.
  • Develops & markets the services of the ACP to promote awareness of and increase utilization of these programs.
  • Coordinates the appropriate ACP Cancer Screenings and educational programs and assists with coordination of all service line cancer screenings to promote an awareness of and increase participation in cancer screenings.
  • Assists with developing and providing in-house and outside community cancer education programs and highlighting ACP programs to address staff & community educational needs related to oncology issues.

  • 3+ years of oncology nursing experience.

  • Graduate of an accredited school of professional nursing. BSN required, MSN preferred.

  • Current R.N. licensure in Commonwealth of Pennsylvania.
  • Preferred OCN certification.
