Outreach Worker

Become a part of the team as an Outreach Worker in the DMH Flex Support Program at Behavioral Health Network!

  • Responsibilities: What you will do
    • Offer structured one-to-one strength-based and resiliency building activities to the identified child or youth in natural community-based settings.
    • Teach social skills, anger management, and other related behavior management skills to youth.
    • Provide care coordination and referrals to community-based agencies, services, and organizations as appropriate.
    • Provide client transportation as needed to address care coordination and treatment needs.
    • Support family in accessing crisis intervention services as needed.
    • Maintain at least weekly contact with DMH regarding the youth's progress.
    • Participate in Community Service Plan meetings with DMH and also works with any behavioral health provider involved with the child in order to support the goals of the service.
    • Coach and supports the youth in expressing him/herself in these planning meetings.
    • Provide daily documentation of his/her interactions with the identified service participant specific to the goal area identified in the treatment plan
    • Provide high quality services to referred children and youth.

Outreach Workers at Behavioral Health Network have the following qualities:

  • Bachelor's degree, High school diploma, or GED required
  • 2-year previous experience working with children with Serious Emotional Disturbance.
  • Good communication skills
  • Ability to create and implement strategies to facilitate achievement of agreed upon goals
