Coordinator - Sepsis

 

HCA Houston Healthcare Clear Lake is a full-service hospital offering a 531 bed inpatient and outpatient medical, surgical and specialty services. The campus includes Level II trauma capabilities, a Neuroscience Institute, The Heart & Vascular Hospital, a bariatrics program, the Breast Diagnostic Center, a wound care center, a level III Neonatal ICU, and the area’s only pediatric emergency department and pediatric ICU.

 

HCA Houston Healthcare Clear Lake is affiliated with HCA Houston Healthcare, part of the HCA Healthcare Gulf Coast Division. The division is a comprehensive network of hospitals, outpatient surgery centers, emergency centers and diagnostic imaging facilities in greater Houston, Corpus Christi and South Texas. What you will do in this role:

To serve as the program coordinator for HHH Clearlake Campus’s Sepsis program. To coordinate organization compliance with industry standards for patients with sepsis, severe sepsis and septic shock. The Sepsis Coordinator is a health care professional who works in collaboration with the medical and hospital staff to provide multidisciplinary care to the septic patient.

• Facilitates development and refines the sepsis protocol to become the standard of care within the facility, while providing and participating in the quality improvement process. 

• The Coordinator provides clinical, professional expertise for patients, families and staff members

• Collaborates with all members of the interdisciplinary team to achieve productive movement of the best practices within the organization

• Provides organized care to the patient with sepsis with a focus on health promotion and disease prevention.

• Functions as an educator and coordinates care in critical care / sepsis patient care management, ensuring optimal patient care management and outcomes.

• Collects and analyzes data surrounding the sepsis patients to assist with documentation, coding, clinical care, and outcomes.

 

 

 

 

Qualifications

What qualification you will need:   Experience:  
  • At least 3 years Acute Care experience required
  • Strong ICU/ED background
  • Must exhibit excellent professional and communication skills to be able to interpret and communicate data, interact with physicians and providers and lead organizational change to improve quality patient outcomes.
Education:                 • RN with Bachelor’s degree in Nursing preferred.   Special Qualifications:          • Current Texas licensure. • Ability to use adult education skills, in order to support learning and informational needs at a level normally acquired through further training in Adult Education or Nursing.  • Ability to multi-task and pay close attention to details and resolve problems. • Intermediate to advanced computer skills, Microsoft, Word, Excel and other database applications.     HCA Healthcare ranks on Fortune’s list of Most Admired Companies for three consecutive years and HCA ranks 63rd on the fortune 500 list.
