Ministry Home Care Plus, an Ascension At Home company, has an opportunity for a

Per Diem Infusion Nurse (RN) located in Marshfield 


Ascension at Home provides comprehensive home-based services across Wisconsin.  Our services include Home Health, Hospice, Home Infusion Therapy, supportive care and emergency response systems.   




Coordinates and performs total nursing care for patients in the home setting; administers skilled nursing care; participates in teaching patients and significant others home medical management; provides leadership by working cooperatively to establish priorities and goals of patient care.  




  • Patient assessment and reassessments
  • Caregiver teaching
  • Development and revision of the Plan of Care and Plan of Treatment
  • Implementation of physician orders
  • Monitoring patient response
  • Response to emergency patient needs
  • Psychosocial support
  • Provides pre-discharge patient assessments when appropriate
  • Completes and submits all required patient care documentation and reports accurately
  • Supports the marketing efforts of the center by assisting the sales team and providing educational programs to referral sources as requested
  • Takes on-call responsibility as assigned to provide 24-hour availability
  • Driving is required to travel to various locations during the work day





  • Graduate of an accredited school of nursing, BSN preferred
  • PICC training desired
  • INS/ONS certification preferred
  • Current state license as a Registered Nurse
  • Continuing education necessary to maintain current licensure required by state or professional organization
  • CPR certification required
  • Valid driver’s license and proof of insurance



  • Minimum of two (2) years current nursing experience in a clinical specialty involving infusion therapy, including patient assessment skills, venous access skills, and emergency patient care
  • A minimum of one (1) year previous specialized home health experience desired


Knowledge and Skills:

  • Excellent interpersonal, communication and organization skills
  • Must be able to assume a flexible work schedule
  • Takes initiatives to present ideas/suggestions to management
  • Communicates to supervisor, physicians and other health care providers, as appropriate, any changes in the patients’ condition
  • Demonstrates clinical competence
  • Participates in the orientation of other nurses or branch personnel as requested
  • Proficient in computer skills (i.e. MS Outlook, Express, Word, Excel, CPR+)
  • Participate in the activities of the Performance Improvement Program
  • Attends scheduled patient care conferences, all in-services, departmental meetings, etc., as requested
  • Communicates with peers, with supervisors and with other co-workers in a professional manner
  • Maintains courteous demeanor at all times
