Social Work Services Coord

Job Summary

Collaborates closely with the multidisciplinary staff to provide a coordinated care experience to patients and families. Receives a high volume of inquiries from a variety of sources and gathers pertinent medical and psychosocial information to help determine level of care need acuity and appropriate resources. Plans and implements patient disposition and other social work case management. Provides utilization management and related service coordination for behavioral health dispositions.

Job Responsibilities

 

  • Participates in the development of the psychosocial support plan. The plan will take into account the needs, wishes, strengths and limitations of the patient, family, and the payer.
  • Able to identify, access and coordinate recommended treatment and service providers within insurance participation guidelines. Conducts insurance verifications and obtains prior authorization of treatment and patient transfers for identified patient population, in accordance with insurance requirements.
  • Establishes relationships with key contacts at managed care organization, service providers and community agencies. Works collaboratively with contacts to ensure patient needs are met. Engages in process improvement work both within the institution and with community partners to provide best possible outcomes for patients and families.
  • Conducts needs assessments and provides support and community-based referrals to patients and families to address identified concerns, including dispersing concrete resources, coordinating appointments, assisting family in obtaining needed documentation, arranging transportation, etc.
  • Partners with team and families to facilitate understanding of care plan and the process for transition to the next level of care. Collaborates with area social workers to provide optimal support, intervention and discharge planning for patients and their families. 
  • Responsible for documenting activities in the medical record.

Required Licenses, Certifications, Registrations

Required Education and Experience

  • Bachelor’s degree in social work required. Bachelor’s degree in a related discipline, such as psychology, behavioral health, sociology, etc. may be considered with relevant experience in social services field.

 

 

Preferred Education, Experience & Cert/Lic

2 years' experience in healthcare or community resource agency preferred.

Additional Technical Requirements

  • Ability to act independently, autonomously and exercise appropriate clinical judgment.
  • Excellent communication, time management and organizational skills.

  • Ability to work effectively with all departments and all levels of professionals.

  • Excellent customer service orientation and strong interpersonal skills.

  • Computer skills that include data input into multiple systems, working knowledge of Word and Excel.
