What Will you be Tasked With?
Provides direct physical therapy according to the physical therapist and in accordance to the Health Care Providers plan of care.
Reports significant changes in the patient’s condition (physical or emotional), safety concerns, unusual occurrences, and adverse effects of medical devices to the physical therapist or nurse manager promptly.
Promotes patient safety and informs the physical therapist and/or designated RN and/or Administrator of any falls or safety problems and documents accordingly.
Provides patients and families with information, support and encouragement to help motivate them in their progress and accepts and adjusts positively to physical, mental, and social limitations.
Demonstrates thorough knowledge of the principles and practices of physical therapy and provides physical therapy assistant services in accordance with the Health Care Provider’s plan of care.
Utilizes physical agents, including but not limited to heat, cold, water, light, electricity, ultrasound and massage as directed on plan of care.
Observes, records and reports to the Physical Therapist, Clinical Manager and Health Care Provider the patient’s treatment, response on a timely basis.
Directs and aids patient/CG in active and passive therapeutic exercises, muscle reeducation, gait and functional training, transfer activities and prosthetic training
Helps the family accept responsibility for providing care and communicates concerns/problems to the physical therapist promptly.
Completes clear concise and accurate clinical notes, which document skilled services appropriately, and submits all requested documentations on a timely basis (24hr).
Participates in performance improvement activities, case conferences, in-services and staff meetings as requested.
Assists physical therapist and/or patients to obtain needed DME/supplies in accordance with the Health Care Provider’s plan of care and recommends safety modifications as indicated.
Uses proper infection prevention and control procedures, universal precautions and safety measures.
Adheres to agency’s policies and procedures.
Provides and documents services in accordance with applicable State, Federal, Medicare, Medical regulations and standards and third party payer guidelines.

Driving or riding in a motor vehicle, standing, sitting, walking, bending, reaching and stretching.

Monterey
Santa Cruz
Watsonville/Hollister
Salinas
King City
Minimum Requirements:
Current license as a Physical Therapist Assistant in the state of practice.
At least one year of work experience under the supervision of a licensed physical therapist. Home Health experience preferred.
Must have a vehicle, current driver’s license and appropriate automobile insurance.
Ability to make home visits and provide physical therapy evaluations and treatments in the home environment, complete documentation effectively and submit in a timely manner.

Home Care Home Base documentation experience
