Job DescriptionPosition/Title: Case ManagerDepartment: Targeted Case ManagementReports To: Targeted Case Management Coordinator/DirectorFLSA Status: Non-ExemptProgram Overview:Our Case Management program is targeted to serve people who suffer from mental health conditions. The program is designed to link each participant to housing, medical and supportive services and agencies in the community who help meet basic needs. Case Management is designed to be a shorter term service which provides a warm handoff to other appropriate services. We create a unique plan of care centered on each person’s unique strengths, needs, abilities and personal preferences.General Description:The Case Manager provides assistance to individuals in the areas of advocacy, service coordination and linkages to community resources. The Case Manager collaborates with community resources to ensure both the mental and physical health needs of the persons served are being met. The Case Manager reports directly to the Targeted Case Management Coordinator/Director.Hours of employment are generally Monday thru Friday, 8:00 am to 4:00 pm, but may vary based on the needs of the program and persons served. Evening, weekend and Holiday hours may be required at times due to crisis situations or emergencies involving the individuals on your caseload.Qualifications:A Bachelor’s Degree in Social Work, Psychology, Rehabilitation Counseling, or a related discipline is required. At least one year of experience working in the human services field. Candidate must have a clean driving record with no more than two points and the ability to pass a criminal background.Job Knowledge, Skills and Abilities:Demonstrated understanding of and willingness to participate in a team environment.Crisis management and de-escalation.Ability and willingness to learn and apply techniques learned in daily work routine as well as crisis management.Demonstrated ability to collaborate with all employees, persons served, and community agencies.Excellent verbal and written communication skills are required.Understand and adhere to agency policies.Must also have a working knowledge of Microsoft computer programs and the ability to use the internet for business purposes.Americans with Disability Specifications (Physical Demands):The physical demands described here are representative of those that must be met by and employee to successfully perform the essential functions of this job. The employees must be able to navigate the physical environment in which the people receiving services live and work.Work Environment:While performing the duties of this job, the employee is exposed to natural weather conditions as this is a mobile job and frequently requires traveling and community visits.The noise level in the work environment is usually moderate.SMCN has an open door policy at all office locations which applies to Supervisors, team offices and individual staff offices. This policy is in place to promote open communication and teamwork within the agency.Essential Duties and Responsibilities may include the following, but are not limited to:Meets program productivity expectation of 75 documented and billable visits per month.Schedules and conducts home visits with persons served to monitor their condition and to assess their needs at least twice per month.Collaborates with persons served to promote and develop partnerships with natural and community supports.Develops an Individual Rehabilitation Plan (Treatment Plan) with the person served to identify case management needs in the areas of advocacy, coordination of services and resource procurement.Utilizes a person-centered approach when providing services.Meets with Supervisor at minimum, monthly for clinical supervision.Documents clinical supervision in the clinical supervision log book.Documents communication and interactions with the person served in the form of encounter notes, assessments collaterals and progress notes in a timely manner.Assures that the assigned cases and all documentation are in compliance and accurate according to agency, COMAR and CARF standards.Provides linkages and access to the necessary resources, entitlements and supports to ensure successful community integration.Identifies with the persons served their barriers to community integration and develops strategies to offset them in consultation with both the person served and their natural support system.Adheres to good ethical practices and establishes professional boundaries with persons served, community resources and co-workers alike.Encourages persons served that are able and interested in working to explore opportunities with the Supported Employment program.Be available for crisis interventions in the form of support while responding appropriately and timely.Promotes Southern Maryland Community Network’s services, develops and maintains positive relationships with individuals, groups and businesses in the community at all times.Confers and advocates with community resources serving the mentally ill including psychiatrists, therapist, mental health agencies and legal services to arrange for needed services in a form that will promote the persons served and their well-being.Assist persons served in completing applications for federal, state and local assistance programs for which they might be eligible for.Assists individuals to make informed choices while following through on responsibilities and advocating for persons served when needed.Demonstrates the ability to communicate clearly and concisely both verbally and in written documentation.Employees are expected to demonstrate cultural awareness by not discriminating against persons served or co-workers based on their age, race, gender, religious preference, sexual orientation, political affiliation, sexual preference, etc.Demonstrates appropriate role model behavior and professionalism at all times.Works within the provisions of HIPAA, SMCN confidentially agreements and/or business agreements.Participates in clinical coordination meetings by actively participating with professional and applicable information as needed to identify and collaborate with other SMCN departments for the well-being and progress of the person served.Provides prompt, dependable and courteous transportation services to persons served on an as needed basis to appointments.Maintains company cars and reports any unusual occurrences and immediately informs the appropriate authority of any incidents, accidents or deficiencies.Properly uses all technology including computers, internet service, phones, fax machines, laptops and cell phones within the confines of SMCN’s policies.Promotes a positive work environment within the Case Management department and the other departments within SMCN.Attends all meetings, trainings as scheduled and directed.Ensures that all policies and procedures set by SMCN and individual programs are up held and enforced.Will ensure that his/her CPR, First Aid and annual training is up to date.Employees are expected to contribute to agency fundraisers by volunteering at events or sitting on the Development Committee.Employees are expected to help SMCN create a healthy and safe work environment by participating on the Health and Safety Committee.Employees are expected to abide by SMCN’s Code of Ethics and report any ethical violations to Human Resources as per policy.All other duties as assigned by Targeted Case Management Coordinator/Director.Job Description Acknowledgement:This job description is intended to describe the general nature and work responsibilities of the position.
