Description
Warwickshire County Council's Hospital Social Care Team is expanding to support older adults in a fast-paced hospital setting. You will manage caseloads, undertake robust assessments and care planning, and collaborate with health partners across Warwickshire.
Based at George Eliot Hospital in Nuneaton, you will work five days per week with some weekend work; a driver’s licence is essential for post-discharge reviews in care homes. Training and supervision are provided by the council.
