Case Manager - Intensive Therapeutic Care

Overview
An exciting opportunity has opened up to join our Intensive Therapeutic Care (ITC) team as a Case Manager on the Central Coast where you will have a real impact making a difference in the lives of children and young people in our care.
In this full-time, permanent role, you will deliver tailored case management to vulnerable children and young people, supporting recovery from trauma and promoting positive life outcomes.
You will work closely with clients, families, and service providers to assess needs, develop care plans, and monitor progress. Flexibility for occasional weekend work and on-call duties is required.
Responsibilities
- Holistic case planning to support safety, wellbeing, and permanency.
- Supporting transitions through and out of care.
- Timely documentation and reporting.
- Building strong relationships with young people, families, and stakeholders.
- Coordinating referrals for after care and supported living.
Qualifications
- Degree in Social Work, Psychology, or related field.
- Experience with trauma-informed care and child protection.
- Understanding of the ITC model.
- Unrestricted driver’s licence and own vehicle.
