Job Description

We’re looking for a vibrant and people-focused Aged Care Engagement and Coordination Officer to join our passionate team in Rockhampton. If you thrive on meaningful connections, love organising, and genuinely care about improving lives this is the role for you.

Responsibilities:

  • Connecting with clients, families, and health professionals
  • Coordinating services and developing individualised care plans
  • Supporting smooth onboarding and ongoing engagement
  • Making every interaction matter

Requirements:

We’re seeking someone who is both caring and capable someone who thrives on connection and brings a high level of professionalism to everything they do. You will ideally have experience in care coordination, case management, or a similar role, and a background in aged care, disability support, or community services.

  • Diploma qualified nurse highly encouraged to apply, RN / EN
  • Certificate IV or Diploma in Aged Care, Disability, Community Services or similar (preferred) with minimum 2 years' experience
  • Knowledge of Home Care Packages (HCP) and or NDIS (Desirable)
