RN - Hospice Admit/Benefits Coordinator

PeaceHealth is seeking a RN - Hospice Admit/Benefits Coordinator for a 1.0 full time equivalent Non-Exempt position. This position will be working Variable shift with shift durations of 8 Hours.

Receive referrals and coordinate history and physical by completing intake information sheet, secure accurate demographics and directions. Review information for appropriateness of referral and confirm insurance coverage. Coordinate admission times.



  1. Coordinate hospice admission activities by assisting in the gathering of appropriate authorization and medical criteria to meet National Hospice guidelines. Demonstrate clinical knowledge of appropriate cause for referral and criteria for admission by evaluating referrals for Hospice suitability/eligibility and discussing with physician. Identify patient's physical, psychosocial and cultural needs in conjunction with referring source. Provide resources appropriate to patient’s age. Set up ancillary services, supplies and equipment needed to maintain patient's safety at home.
  2. Provide care planning/coordination to include coordinate and confirm insurance coverage. Effective planning with appropriate members of the care team. Recommend alternative plan of care to physician and discharge planner if appropriate. Maintain communication with patient care team to facilitate problem-solving about patient issues.
  3. Perform effective communication and documentation by communicating and documenting in accordance with standards. Utilize recognized organization communication standard. Keep abreast of and comply with departmental and organizational changes and events through reading appropriate communications and attending unit and assigned meetings.
  4. Provide education if appropriate to new employees, volunteers, physicians, families, patients and nurses regarding Hospice services utilizing available education resources
  5. Perform general duties to include supporting cost containment and financial viability of the department by meeting productivity standards and by appropriate utilization of supplies and resources. Maintain a working knowledge of infection control, risk management and emergency preparedness plans. Actively participate in performance improvement initiatives. Demonstrate knowledge of hospital, interdepartmental and unit goals, standards and operations.
  6. Perform other related duties as may be assigned.




  • Graduate of an accredited school/college of nursing.
  • BSN preferred.



  • Minimum one year’s Hospice experience (or may have equivalent combination of education and/or experience).



  • WA State RN Licensure.
  • Basic Life Support (BLS) for Healthcare Providers required upon hire.



  • Strong verbal, written and interpersonal communication skills.
  • Strong analysis/problem solving skills.
  • Strong organization skills with the ability to work independently or as part of an interdisciplinary team.
  • Computer skills.
  • Ability to teach.
  • Knowledge of nursing processes and practices.
  • Knowledge of patient care needs in a Hospice environment.
  • Knowledge of National Hospice Guidelines.



  • Work requires moderate physical exertion up to 33% of the time.   
  • Ability to lift objects weighing 30 lbs. or less.          
  • Work is performed under normal working conditions with adequate lighting and ventilation.  Reasonably anticipated exposure to blood and body fluids. 
  • Job duties frequently require intense concentration and attention to detail (34-65% of work time).
