The Licensed Practical Nurse works within the framework of the Patient Care Services Organization as a member of the health care team. The primary responsibilities of the Licensed Practical Nurse is to deliver patient care under the direction of a Registered Professional Nurse within the realm of the New York State Practice Act.
This is part time position, night shift.
Assist patients in reaching outcomes as established by the RPN in the plan of LPN gathers, and documents data related to physiologic, cognitive, cultural, and spiritual status, assists in goal directed plans of care which are evidence-based and age specific, and implements care through utilization and adherence to established standards which define the structure, process, and desired patient outcomes of the nursing process, and communicates information needing referral to RPN and reinforces teaching plan established by RPN.

Primary Responsibilities:
The Driver, Trainee Non-CDL is an entry level driver training role. Associates in this role will learn and/or gain experience in hauling freight and operating tractor trailers while assisting designated driver trainers with their deliveries and routes. -Present Patient Care Form to patients that have proven compliance issues with specific details discussed prior to patient’s discharge
-TCC’s are available to Care Coordination for collaboration on all patients referred to or inquiring HHCAH
-Collaborating and communicating with Primary Care Providers and home care staff to insure continuity of medical care through follow up appointments
-Preparing and maintaining accurate patient records, charts and documents to support sound medical practice
-Notifies appropriate hospital or physician personnel when patient is having difficulty following the transitional care program, helps to identify and remove barrier to goal attainment, and assists with intervention as needed
-Consistently communicates with HHCAH management to make sure all issues and problems are seamlessly handled so that both the patient and the SNF/hospital/physician are satisfied with the results and process.
