Position Summary:

Sovah Health is looking for the ideal candidate who is responsible for assisting patients in overcoming identified or perceived barriers to care.  Prepare weekly tumor conference, manage and maintenance communicate and coordinate EON results for assigned cohorts.  Facilitate Cancer Committee.  Accreditation Program.

·         Assists and provides individualized services to improve overall health outcomes and empower patients to become an active participant in their own care.

·         Provides education, information and resources about the cancer type and it's treatment options at the initial consult and throughout the continuum of care.

·         Assists patients in scheduling tests, procedures, appointments, and treatments and assists patients in coordinating appointments, as necessary, for specialist services and procedures.

·         Reminds patients of upcoming appointments and helps facilitate any transportation needs through available community resources.

·         Refers patients to supportive care services, including dietitians, socials workers, and financial counselors and any other community resource available and appropriate for the patient’s needs.

·         Conducts education and outreach activities and supports needs of the community in relation to cancer education, screening and prevention.

·         Prepare and facilitate weekly tumor conference with pathology and physicians. Maintain required CME and educational requirements for physicians.

·         Coordinate, manage and communicate EON Program results for assigned patients to appropriate physician and navigator.

·         Follow patients through continuum of care from diagnosis through end of life.

·         Facilitate and manage accreditation for cancer program.

·         Conduct quarterly meetings for COC.

Minimum Qualifications:

Minimum Education

High School diploma or equivalent required. Bachelor’s degree in health care, community health, nursing or related human service science preferred. Certification in Navigation preferred.

Required Skills

Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.

Requires flexibility in scheduling and ability to work all shift patterns including week-ends and holidays to insure continuity of care and appropriateness of nursing coverage to the patients of Sovah Health Danville.
