Compensation









Salary Range: $53.65 - $83.58 / hour

The pay range above represents the lowest possible rate for the position and the highest possible rate. Factors that may be used to determine where newly hired employees will be placed in the pay range include the employee specific skills and qualifications, relevant years of experience and comparison to other employees already in this role. Most often, a newly hired employee will be placed below the midpoint of the range.

Pay for PRN positions is calculated differently than Full-Time, Regular Part-Time, and Part-Time positions. PRN Rates depend on the position and other factors, including years of experience, or shift commitment according to the department policy. PRN differential is added in certain positions.















Responsibilities









The Enhanced Care Management (ECM) RN serves as an advocate and assists patients to reach optimal function and health in an appropriate environment that supports their needs. This position provides coordination across the continuum of care, from inpatient to outpatient to community. The ECM RN applies the process of assessment, care planning, implementation, monitoring, evaluation and coordination of care to meet the patient's health care needs across the continuum in collaboration with Primary Care Provider, interdisciplinary care team members, payers, and social service organizations to ensure safe, quality and cost effective care. The ECM RN works closely with individuals to address health disparities, provide health education, ensure closed-loop referrals, and connect people to resources that can improve their overall health and well-being. This position will serve the enhanced care needs of our most vulnerable patient population by meeting them in person, via videoconferencing or over the phone.















Qualifications









Required:

    Current CA Registered Nurse License
  • Three (3) years previous inpatient/outpatient medical case management experience
  • One (1) year direct patient care nursing experience
  • One (1) year of community/population health or related experience
  • Current CA Driver License
  • Current and ongoing Motor Vehicle Report (MVR) that is considered insurable under the CMHS insurance coverages
  • Bilingual in English/Spanish
  • Proficient in Microsoft Office Applications

Preferred:

  • Bachelor's or Master's Degree in a related field
  • Practice experience in evaluation and management of complex medical and psychosocial issues
  • Demonstrated skills in clinical quality improvement and in resource management
  • Previous Project Management and Quality Improvement experience
  • Previous experience in behavioral health
  • Previous Electronic Health Record (EHR) application experience (Allscripts & TouchWorks)
  • Strong analytical/problem-solving, written and verbal communication skills
