Job Summary and Qualifications

The Case Manager PRN (CM) ensures high-quality, patient-centered care by managing Rehabilitative care to ensure optimum outcomes. The CM provides, coordinates, and directs care specific to the needs of each Rehab patient. The CM collaborates with the Rehab Program Director and Facility Case Management Director regarding departmental functions. The CM coordinates efforts within the Rehabilitation team.

What you will do in this role:

  • You will provide program orientation to patients/families/caregivers. Orientation will include the case manager's role, Rehab philosophy, and continued stay and discharge criteria. Orientation will also include Medicare and insurance benefits, grievance procedures, treatment plan process, and rights and responsibilities.
  • You will act as the coordinator of patient/family/caregiver education. You will promote the participation of the patient/family/caregiver in team discussions related to plans, goals, and status. This will be conducted through Family Conferences and other interactions.
  • You will ensure the implementation of the patient's treatment plan that supports the patient's strengths, abilities, needs, and preferences. You will facilitate the involvement of the patient throughout the rehabilitation process.
  • You will document the findings of the Discharge Planning Evaluation (DPE) and psychosocial assessments. You will communicate the social, financial, or discharge needs and preferences of the patient/family/caregiver.
  • You will assume accountability for promoting consistent, positive patient interactions that advance the agenda of unparalleled patient service.
  • Performs continuous assessments and evaluations to ensure patient is progressing towards desired outcome
  • Assesses and responds to patient/family needs by coordinating efforts of other team members
  • Identifies and resolves barriers that hinder effective patient care
  • Maintains confidentiality and complies with HIPPA requirements as it relates to disclosures/transmissions of PHI.
  • Collaborates and consults with physicians, social work, nursing on patient’s progress and discharge planning needs.
  • Coordinates the discharge planning process ensuring involvement of all members of the Behavioral Health team
  • Counsels with patients and family members as appropriate in decision-making and in meeting psychosocial needs of the patient.
  • Maintains knowledge of community resources and facilities available to patients and family members.
  • Collaborates with personnel at other facilities to coordinate timely and appropriate aftercare plans.
  • Compiles, evaluates, and reports statistics to members of the team and utilizes the information to facilitate process improvement.
  • Responsible for the transmission of documents to identified next level of care providers according to Joint Commission and CMS standards.
  • Assists in facilitating safety plan in conjunction with Social Work for each patient prior to discharge.
  • Applies knowledge of the patient population served; applies knowledge of expected norms in planning care
  • Promotes internal and external customer satisfaction.
  • Builds and promotes a culture of service excellence and continuous improvement.
  • Attendance: works as scheduled and is compliant with Attendance and Tardiness Policy.

What qualifications you will need:

Required: Bachelors of Science in Social Work, Social Sciences or Nursing degree from an accredited school

Preferred: Masters of Science in Social Work; previous hospital/community Case Management experience

One to three years of experience in a hospital or health care system

Assertive and persuasive communication skills, action oriented
